A mastectomy is a life-changing decision made for various reasons, including breast cancer treatment, genetic risk factors, or personal choice. Once you and your doctor agree that a mastectomy is the right path, the next step is deciding which type is most suitable for your health, recovery goals, and lifestyle.
At BASS Cancer Center in Walnut Creek, our team of breast surgeons and specialists is here to guide you through every step of the decision-making process.
Types of Mastectomy
Every case is unique. The type of mastectomy your doctor recommends will depend on your cancer stage, tumor size, breast size, skin involvement, and whether you plan to have reconstruction.
1. Simple or Total Mastectomy
- Entire breast tissue is removed.
- Lymph nodes are usually not removed unless involved.
- Chest wall muscles are preserved.
- Often used for non-invasive breast cancer or risk reduction.
2. Modified Radical Mastectomy
- Removes the entire breast and levels 1–2 axillary lymph nodes.
- Does not remove chest wall muscles.
- Common for invasive breast cancer with suspected lymph node involvement.
3. Radical Mastectomy (Rarely Performed Today)
- Removes the breast, all axillary lymph nodes, and chest wall muscles.
- Reserved for aggressive, advanced-stage breast cancer.
4. Partial Mastectomy
- Removes only the cancerous section and some surrounding healthy tissue.
- More extensive than a lumpectomy.
- May still require radiation therapy afterward.
5. Nipple-Sparing Mastectomy
- Removes all breast tissue while preserving the skin and nipple.
- Ideal for small, non-invasive tumors away from the nipple.
- Often combined with immediate breast reconstruction.
Choosing to “Go Flat” After Mastectomy
Many patients choose not to undergo breast reconstruction, a choice known as aesthetic flat closure or "going flat." This option is personal and empowering for many women—and it’s more common than you might think. Studies suggest over 60% of mastectomy patients opt out of reconstruction.
Benefits include:
- Fewer surgeries and faster recovery
- Lower risk of complications
- Ability to wear lightweight prosthetics or none at all
Breast Prosthetics: A Non-Surgical Option
If you choose to go flat but want to maintain your silhouette, breast prosthetics offer a flexible alternative.
Types of Prostheses:
- Lightweight Foam or Polyfill Forms – Ideal for post-surgery healing, workouts, or swimming.
- Silicone Breast Prostheses – Realistic in shape and weight. Available in symmetrical (either side) or asymmetrical (left or right-specific) designs.
These can be worn inside specially designed mastectomy bras.

How to Choose the Right Mastectomy for You
There is no one-size-fits-all approach to mastectomy. When weighing your options, consider:
- Your cancer diagnosis and treatment plan
- Your feelings about body image and reconstruction
- Recovery time and future surgeries
- Support systems and long-term lifestyle
